Title: To Err is Human ... to Learn from Errors Divine

Abstract: I will discuss some of the theory of 'human error' put forth by James Reason in his book "Human Error". Prior theories of human error have grouped errors based on external or contextual properties. Instead, Reason's theory categorizes errors based on the underlying cognitive processes that cause them. Errors are classified into 3 classes: "Mistakes", "Slips", and "Lapses". These classes are closely tied to Jens Rasmussen's "Skill-Rule-Knowledge" framework of learning.
